WHAT YOU'LL NEED
- An Evaluation Request Letter (from your pediatrician or Navigator)
- Any pertinent medical diagnosis/ medical necessity letters from your pediatrician
WHAT TO DO
Submit an Evaluation Request Letter to the Special Education Coordinator at your child’s school.
- Follow up: If you don’t hear back about the special education evaluation request within 5 school days, call the Special Education Department at your child’s school to check on the status.
If your child is NOT enrolled in school and less than 5 years old
What you'll need
- Evaluation Request Letter
- Birth certificate, passport or I-94 card
- Child's immunization records
- Parent/caregiver ID
What to do
Collect the required documents (above) and bring them to the Preschool Office at Garfield Elementary School.
No appointment needed and support is available for Spanish speakers.

What you'll need
- Evaluation Request Letter

- Child's physical exam records
- Any pertinent medical diagnosis/ medical necessity letters
- Parent/caregiver ID
- Past school records / IEP (if available)
- Mortgage statement / lease / affidavit (notarized)
- Utility bill (not cellphone) dated within 60 days
What to do
- Get a registration packet from the Parent Information Center.
- Complete the registration packet and collect required documents (see above).
- Call the Parent Information Center to make an appointment to submit the packet and the documents.
- Follow up! If you do not get a response regarding the special education evaluation request within 5 school days, call the special education department to verify the status of the request.
