If your child is already enrolled in school
WHAT YOU'LL NEED
- An Evaluation Request Letter (from your pediatrician or Navigator)
- Any pertinent medical diagnosis/ medical necessity letters from your pediatrician
WHAT TO DO
- Submit an Evaluation Request Letter from your pediatrician or Navigator to the Special Education Chair at your child’s school.
- Follow up: If you don’t hear back about the special education evaluation request within 5 school days, call the Special Education Department at your child’s school to check on the status.
If your child is NOT enrolled in school
What you'll need
- Evaluation request letter
- Birth certificate, passport or I-94 card
- Immunization records
- Parent/caregiver ID
- School records for student (if available)
- TWO proofs of address:
- Current lease
- Utility bill (not cellphone) dated within 60 days
- W2 Form dated within the year or payroll stubs dated within 60 days
- Bank or credit card statement dated within 60 days
- Letter from government agency dated within 60 days (DTA, DOR, DYS, DCF)
What to do
Submit an Evaluation Request Letter to the Special Education Department:
By email to the Early Childhood Evaluations Contact below. Include child’s name, date of birth, address, parent’s name, phone number, and parent’s concerns for their child. OR
In person (see district address and hours below)
Register your child for school: After the Evaluation Request is processed, you will receive a “registration letter” by mail or email. You will need it to register your child at the Lynn Parent Information Center. Ask the Early Childhood Evaluations Contact to provide it via email if needed.
Note: The process is faster if you submit the evaluation request via email and complete registration online.Once registration is complete, contact the Early Childhood Evaluations Contact to ensure the enrollment has been processed. The timeline for consent writing is based on the date the parent enrolls the child with a “letter of enrollment” from the special education department.
