How to Request a Special Education Evaluation - Everett Public Schools, MA

Parents/caregivers have a legal right to request a free school-based evaluation, free of charge, for their child if they are concerned about any areas of development. Once the school district receives a written evaluation request, they must respond with an evaluation consent form within 5 school days. Here's how to start the process.

If your child is already enrolled in school

WHAT YOU'LL NEED

  • An Evaluation Request Letter (from your pediatrician or Navigator)
  • Any pertinent medical diagnosis/ medical necessity letters from your pediatrician


WHAT TO DO

  1. Submit an Evaluation Request Letter to the Evaluation Team Leader (ETL) at your child’s school.

  2. Follow up: If you don’t hear back about the special education evaluation request within 5 school days, call the Evaluation Team Leader at your child’s school to check on the status.

If your child is NOT enrolled in school

What you'll need

  • Evaluation Request Letter
  • Birth certificate, passport or I-94 card
  • Child's immunization records
  • Child's physical exam records
  • Any pertinent medical diagnosis/ medical necessity letters
  • Parent/caregiver ID
  • Past school records (if available)
  • ONE of the following proofs of residency: 
    • Mortgage statement dated within 60 days
    • Signed Lease and/or Rental Agreement
    • Affidavit
    • Section 8 agreement
    • Property tax bill within the last year with name and address stated
  • ONE of the following proofs of occupancy:
    • Utility bill or work order dated within 30 days
    • Pay stub with name and address stated
    • Excise tax bill with name and address stated


What to do

  1. Call the Parent Information Center and make an appointment.
  2. Collect required documents (see above). Bring these to your registration appointment.
  3. Follow up! If you do not get a response regarding the special education evaluation request within 5 school days, call the special education department.
