How to Request a Special Education Evaluation - Chelsea Public Schools, MA

Parents/caregivers have a legal right to request a free school-based evaluation, free of charge, for their child if they are concerned about any areas of development. Once the school district receives a written evaluation request, they must respond with an evaluation consent form within 5 school days. Here's how to start the process.

If your child is already enrolled in school

WHAT YOU'LL NEED

  • An Evaluation Request Letter (from your pediatrician or Navigator)
  • Any pertinent medical diagnosis/ medical necessity letters from your pediatrician


WHAT TO DO

  1. Submit an Evaluation Request Letter to the Special Education Department at your child’s school.

  2. Follow up: If you don’t hear back about the special education evaluation request within 5 school days, call the Special Education Department at your child’s school to check on the status.

If your child is NOT enrolled in school

What you'll need

  • Evaluation Request Letter
  • Birth certificate, passport or I-94 card
  • Child's immunization records
  • Child's physical exam records, including lead test
  • Any pertinent medical diagnosis/ medical necessity letters
  • Parent/caregiver ID
  • Lease and current rent receipt if lease is older than 1 year
  • One of the following: 
    • Utility bill (not cellphone) dated within 60 days
    • Hospital or Mass Health letter
    • DTA letter
    • Bank statement dated within 30 days
    • Vehicle excise tax statement


What to do

  1. Register your child for school at the Parent Information CenterSubmit the Evaluation Request Letter at the time of registration.
  2. Follow up! If you do not get a response regarding placement within 1-2 weeks, call the Parent Information Center back.
